Functional Capacity Assessments (Adults and Children)
Functional Capacity Assessments (FCA) – $969.95
Australia-wide Telehealth for Adults and Children | starting from $969.95
Compassionate, evidence-based, and accessible Functional Capacity Assessments.
At Seeds Occupational Therapy, we believe every person deserves a high-quality FCA that clearly shows their strengths, daily challenges, and support needs. We keep our pricing fair and transparent — most assessments are $969.95 total, with no hidden extras — so expert occupational therapy is accessible across Australia.
Why choose Seeds OT for your FCA
In-depth, person-centred assessments
Our reports are not generic templates. They are written to reflect your real life, in your own words, and backed by evidence. We use internationally recognised tools such as, but not limited to, the WHODAS 2.0, COPM, PEG, PHQ-9, GAD-7, Katz ADL and Lawton iADL, alongside detailed interviews, document review, and functional observations. This gives NDIS decision-makers a complete and persuasive picture of your daily functioning.
Experienced clinicians who understand complexity
All FCAs are completed by our founder and Principal Occupational Therapist, Nisha Bal, who has over 15 years of experience supporting people with neurodivergence, mental health conditions, chronic illness and physical disability. She understands how multiple conditions interact and translates that into clear, planner-ready language aligned with the NDIS Act (s.24 and s.34).
Telehealth that works for you
We deliver assessments by telehealth to remove barriers like travel, mobility, or health risks. This means you can complete your FCA safely from home, whether you live in the city, regional towns, or remote communities.

Tailored FCA tools — we select the right standardised measures for your situation (e.g. WHODAS 2.0, COPM).
Telehealth interview — an in-depth, supportive conversation where we confirm details and hear your story in your own words.
Report writing — we integrate your voice, evidence from documents, and clinical measures into a clear, planner-friendly FCA.
Recommendations — specific, measurable supports, therapies, and assistive technology linked to your goals.
Important note
Our $969.95 package is for participants who can complete our pre-interview online questionnaires. This allows us to keep costs down while still providing a thorough, high-quality report.
If you are not able to complete these questionnaires, that is absolutely fine — we can adapt the process, but the cost may differ depending on the time required.

Seeds Occupational Therapy is an independent, therapist-led practice with no corporate ownership or franchise structure. This allows us to focus on clinical integrity and equitable access, rather than profit margins.

We also streamline our processes and offer telehealth delivery to reduce overheads.

Comprehensive interviews with you and/or your carers or support people to capture your unique strengths and challenges
Use of standardised and internationally recognised tools, such as WHODAS 2.0, that focus on what you can do rather than just your diagnosis
