How we measure outcomes
Your report includes baseline measures and a simple dashboard so you can show progress. Where suitable, we use tools such as, but not limited to, WHODAS 2.0, COPM, PEG, PHQ-9, GAD-7, Katz ADL and Lawton iADL. We re-use the same tools at review so changes are easy to see and explain.

School or workplace information (for children or people planning return to work/study)
Any incident logs, risk plans, or care plans if you have them
If you do not have these yet, that is okay—we will work with what you have and advise what would help.
Important note about our $969.95 package
Our $969.95 package is designed for participants who can complete our pre-interview online questionnaires. This keeps costs down without compromising quality.
If you are not able to complete the online forms, that is absolutely fine—we will adapt the process (for example, a longer interview or alternative format). The overall cost may change depending on the time required, and we will discuss this with you beforehand.
Children and tailoring the approach
For children and young people, we adapt interviews and tool selection to suit age and developmental stage. We also work closely with parents, carers, and schools to understand daily function across home, school, and community settings.
After your FCA
You receive a clear, planner-friendly report with practical next steps. If something changes or you need clarification for your planning meeting, we can provide a concise summary letter or speak with your support coordinator with your consent.
