Common concerns — answered honestly

Is telehealth rigorous enough for an NDIS FCA?

Yes. We use internationally validated tools including the WHODAS 2.0, COPM, PHQ-9, GAD-7, Katz ADL and Lawton iADL, alongside pre-assessment questionnaires, document review and clinical interview. Many functional difficulties are not visible in a single clinic appointment — they show up in daily patterns, which our structured telehealth process is specifically designed to capture.

My situation is complex — can you handle it?

Nisha has 15+ years of clinical experience with neurodivergence, psychosocial disability, chronic illness, and physical conditions — often in combination. If your case falls outside what we can properly support via telehealth, we will tell you before you commit. We would rather point you elsewhere than provide an assessment that does not properly meet your needs.

The assessment process — step by step

What to expect from first contact to completed NDIS-ready report.

1
Contact us — no referral needed

Fill in the contact form and we will get back to you within one business day. We confirm whether we are the right fit for your situation before you commit to anything. If telehealth is not appropriate for your needs, we will say so clearly.

5–10 minutes No obligation
2
Detailed intake questionnaire

A structured online form capturing your background, goals and daily functioning across all relevant areas. Completed at your own pace from home. A support person, parent or carer can help if needed. This is one of the most clinically important parts of the process — the more honestly and fully it is completed, the richer the report.

At your own pace Support welcome
3
Validated assessment tools — selected for your situation

Our questionnaire covers a comprehensive set of internationally validated tools. Nisha selects the most relevant ones based on your intake — WHODAS 2.0, COPM, PHQ-9, GAD-7, Katz ADL, Lawton iADL and others as appropriate.

Tailored to your case Internationally validated
4
Telehealth clinical interview

An in-depth clinical interview via secure video or phone. Usually 60 minutes. We ask about daily routines, safety, functional variability, informal carer support, harder days, and what happens when support is not available. No travel. No clinic. The interview is focused because background information has already been gathered.

Report writing and delivery

Nisha writes and signs off every report — integrating clinical interview, validated assessment results and supporting documents into a comprehensive, planner-ready FCA. The report explains functional impact, support needs, risk and recommendations in clear, NDIS-aware language. Linked to s.24 and s.34 of the NDIS Act.

most providers?

Seeds OT is an independent practice, owned and run by an occupational therapist. There are no corporate shareholders, no franchise fees, no management layers to fund. Every dollar goes directly into clinical work. That is how we keep fees at the NDIS rate without compromising on quality or depth.

A Functional Capacity Assessment is a detailed occupational therapy assessment that examines how a person manages everyday life — what they can do safely, consistently and sustainably across home, community, work and school. It goes well beyond diagnosis to explain how disability affects real daily function. It is commonly used to support NDIS access requests, plan reviews and support planning.

Two people with the same diagnosis can have entirely different support needs. One may manage many daily tasks independently. Another may need constant prompting, supervision, or assistance just to get through a morning. The NDIS needs functional evidence — not just what condition a person has, but how that condition affects their daily life, participation, safety and the support they actually require.

For most FCA referral questions, yes. A well-structured telehealth FCA uses validated assessment tools, pre-assessment questionnaires, document review, clinical interview and OT reasoning. Many functional difficulties are not visible in a single clinic appointment — they show up in daily patterns across the week, which a structured telehealth process is specifically designed to capture. Where in-person assessment is clinically necessary, we will say so.

Gather relevant documents — GP letters, specialist reports, therapy notes, school reports and previous assessments. Complete the pre-assessment questionnaires carefully and honestly, thinking about harder days rather than only your best days. A carer, family member or support coordinator can assist. Upload documents early. Think about what would happen if your informal support were suddenly unavailable — this is one of the most important things the report needs to explain.

Nisha Bal writes and signs off every FCA — without exception. Interviews may be conducted by experienced OTs within the Seeds team, but Nisha selects the assessment tools, completes the clinical reasoning, and writes the final report in every case. There is no junior staff producing reports without senior oversight.

The telehealth interview is usually approximately 60 minutes. Reports are delivered within two weeks of receiving all information. Current wait time from booking to report delivery is typically 2–6 weeks, and occasionally up to 8 weeks during busy periods. We recommend booking as early as possible to secure your appointment.

No referral is needed. Fill in the contact form — we prefer email because it means we can read your situation properly and give you a more thoughtful reply before you commit to anything. Self-managed and plan-managed NDIS participants are welcome to get in touch directly.
Yes — self-managed and plan-managed participants can claim the assessment cost. We bill at the standard NDIS OT rate of $193.99/hr. If you are agency-managed, contact us to discuss options.
We can email you the forms as a document so you can type your answers at your own pace. If you are unable to complete written forms at all, we can adapt the process with a longer interview instead. This may affect the overall cost, which we will discuss with you before proceeding. Just get in touch and we will find a way that works.

You receive a planner-ready report with support recommendations and an outcome tracking plan. If you need a summary letter for a planning meeting, or if Nisha needs to liaise with your support coordinator, we can arrange that with your consent. The report is designed to be used — not filed away.
